Transitional Care Services get you home fast and safe

For post-acute care, there’s no better place than Glenview Terrace’s Transitional Care Pavilion. It’s where nearly 1,500 people a year turn to for their recovery. One reason, perhaps, is that we get people home in great shape faster than anyone else—approximately seven days for one joint replacement and ten days for bilateral joint therapy.

Seven days a week, 364 days a year—our on-staff, licensed therapists provide one-on-one therapy (very few offer this). And they work with you, your family, your doctor, clinical nurse evaluator and case manager to see you recover fully and speedily. So whether you’re recovering from a joint replacement, stroke, or other ailments—here’s the blend of traditional and advanced rehabilitation services we offer you:

• Mobility skills
• Independence in self care and activities of daily living
• Pain management
• Transition to home and work
• Post-discharge equipment needs
• Medical needs such as IV antibiotic treatments, wound care, diabetic management and respiratory services
• Patient and family education
